We are starting accreditation for the pre-season game CSKA Moscow – Utah Jazz which will take place in Universal Sport Hall CSKA on October, 8, 2004. A two-page credential application form must be completed in order for the request to be reviewed.

A two-page credential application form must be completed in order for the request to be reviewed. Both pages of the application must be completed and signed in order to be considered for approval. You can send application from through fax +(095) 250-7796 or bring them to office of PBC CSKA . All credential application forms for Utah Jazz vs CSKA Moscow must be received by September 8, 2004.
